MY VAX INDIANA

Indiana 2023-24 Required & Recommended School Immunizations

The Indiana Department of Health (IDOH) maintains the Immunization Information System, CHIRP (Children and Hoosier Immunization Registry Program). Through the MyVaxIndiana online portal, Hoosiers can access their family’s vaccination records from any computer.

To access your or your family’s vaccination records:

Request your Patient ID Number (PIN) from your medical provider or local health department.
Click here
Select Search from the menu bar
Enter your information, PIN, and review the record release statement
Select the Get MyVaxIndiana button
You can then print or download the vaccine record.

Please call the Henry County Health Department at (765)521-7059, Extension #3, to schedule your appointment today or let your health assistant/Corporation RN know if your student needs an appointment. We will help you schedule one at school or the health department.

Current School Immunization Requirements

Tdap Vaccine Information (Tetanus, Diptheria & Whooping Cough)

All preteens should get one Tdap shot when they are 11 or 12 years old to help protect against tetanus, diphtheria, and whooping cough. Contact your child’s doctor if they haven’t gotten this vaccine yet.

Meningococcal Vaccine Information

Meningococcal vaccine protects against a type of bacteria that can cause serious illnesses. The two most common types of illnesses are meningitis and bloodstream infections. All children ages 11 through 12 years should receive a meningococcal vaccine followed by a booster dose at age 16 years.

Looking up nutrition information BEFORE you dine out can help you make healthier choices away from home. Check restaurant websites, phone apps, and menus. Once you have the nutrition information, choose items lower in calories, saturated fat, trans fat, sodium, and added sugars.

If you use tobacco and also have a child who vapes, the best way to help support your child is to quit tobacco use yourself. There are many free resources available to help you start your quit journey whenever you are ready.

If you are an adult looking for support to quit vaping or stop using any tobacco product, the free digital quit smoking platform, BecomeAnEX, offers comprehensive web and mobile tools. Parents of vapers can also sign up for BecomeAnEX, where they can indicate that they are a parent of a young person who vapes, and access a supportive online community and a full interactive website of information about how nicotine addiction works and different approaches to quit.
